Requirements for Organizing an Iraqi Death Certificate

The Consular Section in Washington issues Iraqi death certificates for Iraqis who passed away in the states within its jurisdiction: Alabama, Connecticut, Delaware, Florida, Georgia, Maine, Maryland, Massachusetts, New Hampshire, New Jersey, New York, North Carolina, South Carolina, Pennsylvania, Rhode Island, Tennessee, Vermont, Virginia, and West Virginia.

For states outside this jurisdiction, please contact the Consulate General responsible for that specific state.

Required Documents & Steps

Electronic Form: Fill out the electronic application form for a death certificate via this link:

Appointment: Book an electronic appointment through the same link mentioned above.

U.S. Death Certificate: Provide the original U.S. Death Certificate, authenticated/apostilled by the Secretary of State in the capital of the state where the death occurred.

Iraqi Identification: A copy of the deceased’s Civil Status ID and Iraqi Nationality Certificate, or the Unified National Card.

Submission: The application can be submitted in person at the Consular Section in Washington or sent by mail. If mailing, include a pre-paid self-addressed return envelope to ensure the documents are returned to you after processing.

Shipping Methods: Please use USPS Priority Express, UPS, or FedEx.

Fees and Late Penalties (Law No. 21 of 2024)

Note: Law No. 21 of 2024 regarding death registration went into effect on September 23, 2024. Late penalties for deaths occurring before this date follow the previous regulations.

Within 30 days of death: Issuance is Free.
After 30 days up to 6 months: A one-time late fine of $20 USD.
After 6 months: A late fine of $58 USD for every month following the initial six-month period.
Repatriation of Remains: A fee of $3,200 USD is charged for transporting the remains of Arabs or foreigners for burial in Iraq.
