Requirements for Issuing a Life Certificate (Retirement)
The Consular Section in Washington issues Iraqi Life Certificates for Iraqis residing in the following states: Alabama, Connecticut, Delaware, Florida, Georgia, Louisiana, Maine, Maryland, Massachusetts, Mississippi, New Hampshire, New Jersey, New York, North Carolina, South Carolina, Pennsylvania, Rhode Island, Tennessee, Vermont, Virginia, and West Virginia.
For residents of other states, please contact the General Consulate responsible for your state.
I. Organizing a Life Certificate (Retirement) via Regular Mail
Instructions for those with Iraqi Identification Documents:
Form: Fill out and sign one "Retirement Life Certificate" form (no notary or tax office endorsement required). Send it via regular mail with a pre-paid return envelope to the mission. Include copies of your ID documents, retirement ID, and a fee of $20.
Video Call: After you receive the official form with a barcode back from the embassy, you must conduct a video call with the Consul for signing and thumbprinting. This occurs during business hours (9:00 AM – 2:30 PM EST). You must have a blue pen and a blue ink stamp pad ready.
Return: After the video call, mail the two signed certificates back to the Consular Section with a pre-paid return envelope. The embassy will then stamp, sign, and return your copy.
Documents Required:
- Copy of Retirement ID.
- Copy of Civil Status ID, Iraqi Nationality Certificate, Unified National Card, or a valid Iraqi Passport.
- Two (2) recent personal photos with a white background.
- $20 USD cash.

II. Organizing a Life Certificate (Retirement) via In-Person Visit
- Personal presence of the retiree is required.
- Copies of identification (Retirement ID, Civil Status ID, Nationality Certificate, Unified Card, or valid Passport).
- Two (2) recent personal photos (white background).
- Fee: $20 USD.
III. Life Certificates and Written Declarations for Power of Attorney (POA)
These transactions generally require in-person attendance.
- Copies of Iraqi identification (Civil Status ID, Nationality Certificate, Unified Card, or valid Passport).
- Photos:
- Three (3) photos of both the principal and the agent for General POAs and Real Estate POAs.
- Two (2) photos for the principal for other types of POAs.
- Copies of the agent's Iraqi ID and a copy of the POA to be acknowledged.
- Fee: $20 USD.
